Welcome to Chatham-Kent’s Supports Referral System
Chatham-Kent Support Referral System is a single point of access to help families contact the services and programs they need and want.
There are 4 easy steps to get you started:
Step 1: Click "Create a Profile"
Step 2: Complete your Family Information for you and your child
Step 3: Indicate which supports you are seeking for your child
Step 4: Sign the electronic consent form that you authorize your child's information to be shared with the appropriate community partner agencies.
A secure electronic referral will then be submitted to the appropriate community partner. Follow-up contact will be made with you regarding your submission.
