Notice of Collection of Personal Information Information is collected pursuant to the Child and Family Services Act R.S.O 1990, c.C1, the Municipal Freedom of Information and Protection of Privacy Act, the Personal Health Information Act, 2004, the Municipal Act, 2001 and the Day Nurseries Act, R.S.O. 1990 and will be retained, used, disclosed and disposed of in accordance with the legislation and the nature of the information collected. Information will be shared will all affiliated agencies selected by the legal guardian, for the purposes of requesting services from partner agencies or for placing your child in an available child care . If your child has attended a child care centre in the past, information regarding outstanding child care fees may be shared between child care operators before your child is placed again.
